HSE invite those with long-term health conditions to attend free self-management programmes

Are you an adult living with a long-term health condition, or are you a carer for an adult with a health condition?

The HSE in South Donegal will be hosting a showcase of Self-Management Programmes for sufferers of long-term health conditions such as Diabetes, COPD, Asthma, Stroke, Heart Disease, Arthritis, Depression, Multiple Sclerosis, Parkinson’s disease and many others.

A number of these programmes also support carers.

The showcase takes place next Wednesday, 22nd November, from 10am to 2pm at An Clochar Health Campus, College St, Ballyshannon.

It is open to the public, who can call in at any stage during these hours to find out information on a range of Self-Management Programmes.

The objective of these Self-Management Programmes is to increase people’s skills and confidence in managing aspects of their long-term health condition. This is achieved by giving people access to the right information, education, and support.

All the programmes are free.

Programmes that will be represented include:

    • DESMOND, for people living with Type 2 Diabetes
    • CODE, for people living with Type 2 Diabetes (delivered by Diabetes Ireland)
    • Stanford ‘Quality of Life’ for people living with any long term health condition
    • Confidence to Care, for people living with or caring for someone with a life-limiting condition
    • Social Prescribing: Connecting people with non-medical community supports
    • Smoking Cessation
    • Falls Prevention
    • Stress Control (delivered by Donegal Mind Wellness)
    • Continence Care
    • Carers/Stroke Carers

Some of these programmes are relevant to specific health conditions and others are generic, meaning they offer support for the challenges commonly met by people living with any long-term health condition.

All are welcome, so come along, get the information you require and meet with the health professionals involved.
